Terms and conditions: 

• Must be US resident 18 years or older.
• You must be a previous client that has completed surgery with TreVita to enter into our Referral Program
• Void where prohibited.
• There are no limitations on the number of referrals you can make.
• This offer is not retroactive and cannot be combined with any other ongoing promotions or offers. We are committed to providing an exceptional experience and ensuring that each participant receives their well-deserved rewards.
• TreVita makes all determinations regarding participation in the program. We reserve the right to terminate the program, or your participation in the program at any time in our sole discretion, to the extent permitted by law, upon notice to you.
• We prioritize your privacy in our Referral Program. We collect your information for program purposes, ensuring the smooth management of referrals and providing timely updates. Rest assured that your data remains secure and is not shared for marketing purposes. You have the right to access and manage your information, and please note that our privacy statement may be subject to periodic updates. Your privacy is of utmost importance to us, and we appreciate your trust in our Referral Program.
• Referring part agrees to Defend, indemnify, release and hold harmless TreVita its parent companies, affiliates and subsidiaries, together with their respective employees, directors, officers, members, attorneys and agents including, without limitation, their respective advertising and promotion entities and any person or entity associated with the production, operation or administration of the Program (collectively, the “Released Parties”), from any and all claims, actions, demands, damages, losses, liabilities, costs or expenses caused by, arising out of, in connection with, or related to Referrers’ participation in the Program (including, without limitation, any property loss, damage, personal injury or death caused to any person(s) and/or the awarding, receipt and/or use or misuse of the Program or any Reward.
